We are seeking a strategic and collaborative professional to lead integrated community care initiatives supporting seniors within a defined sub-region.
This role focuses on programme strategy, cross-sector partnerships and performance management to ensure seamless, coordinated care delivery across healthcare and community providers.
You will play a pivotal role in aligning stakeholders, strengthening care pathways and driving measurable impact on seniors' health and well-being.
Lead planning and implementation of integrated care programmes within the sub-region
Collaborate with community care providers to co-develop shared care plans using standardised assessment tools
Design workflows to ensure seamless referral and care transitions
Optimise resource allocation for efficient and coordinated service delivery
Establish joint accountability frameworks to meet KPIs and service agreements
Build and manage partnerships with healthcare institutions, primary care providers and community agencies
Align goals and outcomes across partners to improve senior health outcomes
Establish data-sharing mechanisms across providers
Facilitate collaborative engagements and manage stakeholder communications
Monitor programme KPIs and funding requirements
Track and manage programme budgets
Prepare and vet progress reports with accurate data
Ensure compliance with PDPA and regulatory standards
Recommend policy and process improvements
Degree from an accredited institution
Minimum 5 years of experience in social services, healthcare, programme management or community care
Experience working with healthcare institutions or multi-stakeholder environments preferred
Strong stakeholder engagement and partnership-building skills
Strategic thinking and programme management capability
Excellent reporting and communication skills
Analytical and solutions-oriented mindset
Ability to manage complexity across multiple providers
