Social Worker (Mental Health & Eldercare - Community Services)

When a family faces a new dementia diagnosis, or an individual struggles to manage a severe mental health condition alone, navigating the social support ecosystem can feel overwhelming. As a Social Worker with us, you won't just handle cases-you will be the steady guide, counselor, and advocate who helps individuals and caregivers rebuild stability, dignity, and hope.

In your first 90 days, you will step directly into the community: carrying an active casework load, conducting psycho-social assessments, and building tailored intervention plans. You'll collaborate closely with healthcare professionals, community partners, and caregivers to ensure no client falls through the cracks.

What You Will Do

• Casework & Psycho-Social Assessment: Gather holistic client backgrounds, evaluate psycho-social needs, and co-create actionable intervention goals with clients facing mental health challenges, dementia, or complex life transitions.

• Counselling & Caregiver Support: Provide direct therapeutic counselling and continuous emotional and practical support to both individuals and their primary caregivers.

• Community Outreach & Group Work: Plan, organize, and execute community-based outreach initiatives, preventative programs, and support groups tailored to community needs.

• Information & Referral (I&R): Map out and connect clients to appropriate social service agencies, healthcare providers, and financial assistance schemes.

• Programme Evaluation & Research: Monitor client outcomes, prepare comprehensive reports for intervention programs, and participate in service-evaluation research to continuously refine community care standards.

What We Are Looking For

• Educational Qualification: Bachelor's Degree in Social Work OR a Bachelor's Degree (any discipline) paired with a Graduate Diploma in Social Work from an accredited institution.

• Professional Accreditation: Registered Social Worker (RSW) status with the Singapore Association of Social Workers (SASW) is highly preferred (or eligible for registration).

• Core Competencies:

  • Strong analytical and problem-solving skills to evaluate complex family dynamics quickly and formulate clear intervention recommendations.

  • Excellent written and oral communication skills for case documentation, report writing, and stakeholder collaboration.

  • Demonstrated empathy, resilience, and a collaborative team-first mindset.

• Experience: Prior social work experience-specifically within mental health, eldercare, or community care-is a strong advantage, though dedicated mid-career entry candidates with the required qualifications are welcome.
